Umm. Well I use photoshop and the pencil tool on 1px and just dot them out really starting with an outline then filling in the colours and such.

Idk how to explain really; if you have any specific questions I'll be able to answer them better

Hehe well alot of people use Microsoft Paint as they think Photoshop can be regarded as a cheat way because of the special features. There's always the pencil tool on Gimp (that's a free program and I downloaded it the other day and it appears to work fine)
